Flat Rock Care Center
Flat Rock Care Center accepts children ages 6 years to 18 years. While residents may remain in our care until they are 21 or older, we make it our policy to admit children under the age of 18.
Those interested in discussing intake at Flat Rock should contact the Coordinator of Program Services, Kim Lee (email@example.com ) at 419-483-7330 (ext. 1109). Families, Service and Support Administrators from County Boards, Members of Local Churches and anyone needing care, should feel encouraged to reach out to us.
The intake process includes application materials, intake meeting with the prospective resident, parents/guardian(s), Service and Support Administrator and FR Interdisciplinary Team (IDS), an on-campus pre-admission visit of 5-7 days, and a formal recommendation of the IDS team to place the prospective resident on our waiting list if appropriate.
Flat Rock Care Center currently has a waiting list for males and females.

Flat Rock Community Services
Flat Rock Community Services works with area counties to provide supported living environments in local communities. Intake for our Community Services program is typically on a referral basis from our county partners. However, due to our dedication to christian service and values, we make every effort to serve those who need care.
